Athletes will need to bring a completed Physical packet when reporting for their physical. Physical forms are available by using the links below. Athletes that did not participate in a Fall Sport will need to download and complete Sections 1-3. You will only need to download Section 4 if you are receiving your physical from your personal physician.

Athletes that competed in a Fall Sport will only need to complete Section 5.

Unfortunately, make-up physicals will not be scheduled. Any athlete that is not available on Friday will be required to receive their physical from their family physican.

All forms must be completed and submitted to the Athletic Trainer before the athlete will be permitted to participate in practices.
